To file a Professional Liability Claim with Starr Wright USA, you must send us a written claim – you can submit the form by mail, fax, or email, and must include the following information at a minimum:
- Your full legal name;
- Your home address;
- Your employing federal agency;
- Your Starr Wright USA Member ID number;
- Your personal telephone number and email address that we can use to contact you;
- A brief description of the circumstances that give rise to the claim along with supporting documentation (memoranda, emails, a notice of adverse action, a notice of investigation, civil complaint, subpoenas, interrogatories, etc.);
- Whether you have a union or other employee association representation or you have other professional liability insurance coverage;
- Whether you currently are represented by an attorney; and
- Your signature.
