Do you need to report a Professional Liability Claim?

To file a Professional Liability Claim with Starr Wright USA, you must send us a written claim – you can submit the form by mail, fax, or email, and must include the following information at a minimum:

  • Your full legal name;
  • Your home address;
  • Your employing federal agency;
  • Your Starr Wright USA Member ID number;
  • Your personal telephone number and email address that we can use to contact you;
  • A brief description of the circumstances that give rise to the claim along with supporting documentation (memoranda, emails, a notice of adverse action, a notice of investigation, civil complaint, subpoenas, interrogatories, etc.);
  • Whether you have a union or other employee association representation or you have other professional liability insurance coverage;
  • Whether you currently are represented by an attorney; and
  • Your signature.
